Home Health RN Case Manager — Patient Care Leader

Caretenders Home Health, part of the LHC Group within the Optum family, is hiring an RN to deliver in-home nursing care across Kentucky. You will operate per the plan of care, coordinate referrals, determine benefits, and ensure care meets physician orders while maintaining patient-centered service. Requires current KY RN licensure, valid driver's license, reliable transportation, and willingness to travel to patient homes.
