Weekend Admission Nurse (RN) - Home Health

Part-time Registered Nurse Case Manager Anticipated Territory: Sacramento County Schedule: Every Saturday and Sunday Full Days, or Saturday/Sunday plus 3 weekdays as needed. Please note: All opportunities at 21HHS require field visits to patients' homes. What you’ll do Conduct Start of Care (SOC) admissions and OASIS assessments. Perform follow‑up, evaluation visits, and home visits within the designated territory. Reconcile medications, provide wound care, and perform treatments and procedures. Monitor clients’ conditions and report changes to the clinical team. Collaborate with physicians and agency staff to deliver comprehensive care. Initiate and administer IV medication infusions (antibiotics, diuretics) following established protocols. Accurately document observations, interventions, and evaluations related to patient care. Competitive base compensation with incentive bonus program. Qualifications Current and unrestricted California RN license. Current BLS/CPR certification. Valid California driver’s license. Experience with Start‑of‑Care (SOC) assessments and OASIS (highly desired). High level of clinical knowledge in home‑health care (preferred). Experience with high‑acuity patients (plus). Experience working with interdisciplinary teams (desired). Case‑management fundamentals (desired). Experience with IV medication infusions (highly preferred). Proficiency in initiating intravenous therapy (plus).
