We are looking for a Full-Time Licensed Practical Nurse (LPN) to join our team in Somerset County. This position would be responsible for providing quality care and home health nursing care services under the direction of a Registered Nurse or an Advanced Practice Nurse.

ResponsibilitiesWhat You'll Do:

• Bringing care plans to life under the guidance of an RN

• Teaching and supporting patients with preventative and recovery care

• Ensuring services meet the highest standards of quality and compassion

• Collaborating with doctors, nurses, and other care professionals

• Maintaining patient safety, documentation, and infection control standards

• Helping patients regain independence and dignity—one visit at a time

QualificatonsWhat You’ll Bring:

You’re not just a nurse—you’re a difference-maker. Here’s what we’re looking for:

• A graduate of an NLN-accredited LPN program with an active NJ LPN license

• 2+ years of med/surg or acute care experience (homecare/hospice a plus!)

• Strong communication, time management, and tech skills

• A valid driver’s license, reliable transportation, and a willingness to travel

• A heart for care and a passion for helping others thrive

Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $26.00 to USD $34.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
