Speech Language Pathologist

Speech Language Pathologist (SLP) – We are seeking a Speech Language Pathologist to deliver skilled speech therapy services to patients across Senior Living Communities at Covenant Home Health Agency (CHH). The role includes administering therapy in patients’ residences in accordance with physician orders and the overall care plan, under the supervision of the Director of Clinical Services/Clinical Manager. Responsibilities Provides full‑range Speech Language Pathology services as ordered by the physician. Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function. Identifies functional speech defects and establishes a plan of care to improve the patient’s function. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment. Recommends communication devices/aids as indicated. Completes initial evaluations, including a plan of treatment and goals, and submits to physician for approval; maintains recertification with physician every 60 days if treatment continues; writes progress notes at each visit; writes reevaluations at expected treatment end; writes summary upon discharge. Helps develop and revise the plan of care as necessary. Consults with physicians regarding changes in treatment. Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care. Participates in case conferences, in‑services, and other meetings as required to ensure coordinated and comprehensive care. Prepares and submits clinical and progress notes per organization policy. Maintains clinical competency in speech‑language pathology practice and theory. Identifies patient and family/caregiver needs for other services and refers as appropriate. Instructs patient, family/caregiver, and other organization health‑care personnel in the patient’s treatment regimen as indicated. Conducts a supervisory visit every fourteen days on each patient seen by a home health aide as assigned. Completes mandatory CEU’s to maintain professional licensure. Understands and adheres to established policies and procedures. Completes Christian Care onboarding as well as Covenant Home Health orientation. Licensing & Qualifications Possesses a master’s or doctoral degree in speech‑language pathology (SLP). Is licensed as a speech‑language pathologist by the State in which the individual furnishes such service, OR Has completed 350 clock hours of supervised clinical practicum (or is in the process of completing), at least nine months of supervised full‑time SLP experience, and has successfully completed a national examination approved by the Secretary. A minimum of one (1) year of clinical experience in Speech‑Language Pathology. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements. Schedule and Service Areas Full‑time & PRN to cover the West Valley. Part‑time PRN to cover the East Valley. Daily pay option available. Medical, dental, and vision insurance options.
