Position Summary:

The Family Case Manager provides family-centered case management and supportive services to children and families participating in St. Monica’s programs and Family Drug Court. This role supports child and family well-being through parenting support, coordination of services, participant education, advocacy, and collaboration with treatment teams and community partners. The Family Case Manager promotes culturally responsive, trauma-informed, and developmentally appropriate services that support healthy family functioning and positive child outcomes.

Core Responsibilities:

• Provide case management and supportive services related to children and family needs within residential services and Family Drug Court. 
• Support participants in strengthening parenting skills, family relationships, and healthy child development practices. 
• Facilitate parenting, psychoeducational, and child development programming as assigned. 
• Coordinate services and communication related to children’s needs with internal treatment teams, community providers, and Tribal entities as applicable. 
• Support participant visitation, family engagement, and reunification efforts in a culturally responsive and participant-centered manner. 
• Monitor and communicate concerns related to child well-being, development, safety, and family functioning toappropriate treatmentteam members. 
• Support implementation of evidence-based and culturally responsive parenting and family programming. 
• Fulfill requirements of FamilyDrug Court contract.
• Assist with participant advocacy, resource coordination, and connection to community supports and services.

• Group Facilitation: Facilitates parenting and psychoeducational programming in an engaging and supportive manner. 
• Cultural Responsiveness: Incorporates culturally responsive and trauma-informed approaches into participant services. 
• Collaboration & Care Coordination: Communicates effectively with treatment teams, community providers, and external agencies. 
• Documentation & Compliance: Completes accurate, timely, and compliant documentation. 

Education and Experience:

• Associate’s degree in psychology, sociology, human services, or a related field preferred. Two years of relevant coursework and/or two years of experience in substance use, mental health, case management, or related human services may be accepted.
• Experience working with children, families, substance use, and/or mental health services preferred. 
• Knowledge of child development, parenting education, and community resources preferred. 
• Experience working with individuals and families from diverse backgrounds preferred. 
• Must be at least 21 years of age. 
• First Aid/CPR certification required or ability to obtain. 
• Valid driver’s license and proof of insurance required upon hire. 

Physical Requirements:

Ability to perform the essential functions of the position, with or without reasonable accommodation. This may include lifting up to 25 lbs., climbing stairs, working in community settings, and working in a fast-paced, participant-centered environment.
