What You’ll Do

As a Licensed Practical Nurse (LPN), you’ll provide safe, compassionate, and high-quality nursing care in accordance with physician orders, facility policies, and applicable state and federal regulations.

Your responsibilities will include:

• Provide nursing care within the scope of your LPN license and established standards of practice.

• Monitor residents and promptly document and report changes in condition, including nutritional concerns, infections, skin conditions, weight loss, and other clinical changes.

• Complete accurate and timely charting and incident reports.

• Follow established procedures for medication management, narcotic counts, and pharmacy orders.

• Support restraint-reduction efforts and follow applicable facility procedures.

• Utilize the Quality Life Manual to identify, place, and monitor residents in appropriate programs.

• Communicate resident room changes, leaves of absence (LOA), admissions, and discharges to the appropriate departments.

• Participate in physician rounds as needed.

• Collaborate with the interdisciplinary team to support resident safety, comfort, and quality of life.

• Follow all applicable facility policies, state regulations, and federal guidelines.

Qualifications

• Current Licensed Practical Nurse (LPN) license in the state of practice.

• Current BLS/CPR certification or willingness to obtain certification.

• Strong communication, organization, and clinical documentation skills.

• Compassionate approach to resident care.

• Ability to work collaboratively with residents, families, and the healthcare team.
