The position offers independence, flexibility, and the opportunity to provide personalized care that honors each patient’s goals and quality of life. What You'll Do Complete comprehensive patient assessments and ongoing evaluations Develop, implement, and revise individualized care plans Manage pain and symptoms to maximize patient comfort and quality of life Provide skilled nursing services in accordance with physician orders and hospice regulations Educate patients, caregivers, and families regarding disease progression, medications, and care planning Coordinate care with the interdisciplinary hospice team Serve as a clinical advocate for patients and families Provide emotional support and guidance during challenging times Maintain accurate and timely documentation Participate in interdisciplinary team meetings and care conferences Ensure compliance with agency policies, Medicare regulations, and hospice standards of practice The Ideal Candidate Leads with compassion and empathy Enjoys building meaningful patient and family relationships Thrives in an autonomous field‑based environment Is passionate about patient‑centered care Communicates effectively with patients, caregivers, physicians, and team members Finds purpose in helping patients live with dignity, comfort, and peace Required Qualifications Current California Registered Nurse (RN) license in good standing Minimum of one year of nursing experience Valid driver’s license, reliable transportation, and current automobile insurance Strong assessment, clinical judgment, and critical‑thinking skills Excellent communication and organizational abilities Preferred Qualifications Hospice experience Palliative care experience Home health experience Case management experience Experience working independently in the field Hospice experience is preferred but not required.
