Home Health Aide

As a Home Health Aide with Family Support Care, you will play a vital role in delivering high-quality, compassionate care to our patients in the comfort of their own homes. With our team, you will have the opportunity to build strong relationships with your patients and their families, providing personalized support and assistance to enhance their overall well-being.
Responsibilities:
• Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
• Assist with meal preparation, light housekeeping, and laundry
• Promote patients' independence and dignity while encouraging social interaction and community engagement
• Monitor and report changes in patients' condition to healthcare professionals
• Maintain accurate records of patient care and services provided
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Provide emotional support and companionship to patients and their families
About the Role:
As a Home Health Aide, you will have the flexibility to work in a variety of settings, including private homes, assisted living facilities, and skilled nursing facilities. You will have the opportunity to work with patients of all ages and backgrounds, and will be provided with ongoing training and support to ensure your success in this role.
