Department:SOM Wichita Pediatrics-----Pediatrics - Baby TalkPosition Title:Family Support Advocate - Spanish (Baby Talk Program)Job Family Group: Professional StaffJob Description Summary:Baby Talk Family Support Advocate will provide support for the Baby Talk community collaborative prenatal education program. The Baby Talk Family Support Advocate is at the heart of our program, taking care of everything from building relationships with clinical sites to facilitating engaging virtual and in-person classes. Our mission is to empower birthing persons to advocate for themselves and their babies, improving birth outcomes and baby survival rates. This position will assist pregnant persons attending Baby Talk classes from referral through six-week post-partum, ensuring they receive the support they need. The Family Support Advocate will collaborate with other maternal child health programs and address social determinants of health to create a holistic support system. This position will create positive experiences for our participants, proactively engage with families to overcome barriers, and connect them with valuable community resources. This role requires a high level of integrity, professionalism, and problem-solving skills when working with participants, educators, and community partners. This position will pay $21.63 an hour. A cover letter and resume are required to be considered for this position.Job Description:Job Duties:Program SupportServe as the liaison between the partnering sites, volunteers, clients, the Program Director and the Baby Talk teamCall, track, and follow-up on referrals received through the website and community partnersFacilitate class logistics for both in person and virtual classesCreate client profile and collect necessary intake forms in the electronic data system (e.g. Nightengale Notes) or in-personSchedule clients to each class in the electronic data systemMonitor client attendance to ensure completionProvide communication to clients, educators and those involved in classes regarding the scheduleMonitor supplies and needed materials (maintaining inventory) at class locations including client paperwork, class materials, educational support materials, and updated resource brochures and handoutsUse the standardized reminder systems for all participants (e.g. text messages, reminder calls, Teletask reminders)Provide consistent communication to the Baby Talk instructors and Baby Talk administrative team to share updates related to the program and ensure issues are addressed and progress is madeRemain up to date on community resources and services provided to facilitate meeting Baby Talk participants’ needs outside of what the program providesMaintain a set schedule of hours at clinical locations to foster relationships and provide program-related supportProvide backup and assistance for Sedgwick County and statewide Baby Talk classes and participants assigned to other staff membersProactively promote the Baby Talk program within partner sites and to other community organizationsEngage in program outreach activities, which may include occasional weekend or evening hoursSupport special projects to improve maternal and infant health including promoting research opportunities to program participantsCase Management Identify and work with clients to resolve barriers to program participationFollow up with clients with positive screening resultsIdentify client needs outside of the scope of the Baby Talk programCoordinate connections to other programs to meet the needs of the clients, including: • Referring through established partnerships with area programs • Ensuring follow-through with enrollment, participation as needed • Provide assistance to navigate through various systems related to access to services • Identify barriers and assist clients in strategizing to overcome these obstacles (e.g., transportation, childcare)Establish, facilitate, and follow-up on client referrals to other community/clinic programsReport issues outside of scope to Program ManagerData Collectionand EntryObtain necessary authorizations and consent from clientsCollect initial, completion and birth outcomes or other required program data directly from clientsEnter data relative to the program from all sitesAdministrative and Grant Support Attend bi-weekly CRIBS program and Baby Talk staff meetingsAssemble class binders and ensure adequate supplies and materials are available for participantsAttend Baby Talk site meetings and assisting with schedulingAttend all KDHE required trainings, webinars and conferencesAssist with the completion of quarterly grant update reports and annual grant applicationsParticipate in collaborative work with other state-wide sites, including attendance at relevant conferences, trainings and workshopsThis job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. If assigned to a partnering clinic, the position will include scheduled time at that location.Required Qualifications:Education: High school diploma/GEDWork Experience:Bilingual in SpanishExperience with databases, email, the internet, Zoom, Teams, Microsoft Word, Excel, and PowerPointPreferred Qualifications:Education: Bachelor’s degree in health, social work, psychology or related fieldWork Experience:2 years’ experience working in one of the following areas: with community-based programs, heath care systems, health departments, or clinicsExperience with maternal infant health and other support resources available in the communityExperience scheduling and coordinating appointments, classes, or servicesExperience with data collection and entryExperience working with the KanCare system and/or the uninsured populationExperience with the Integrated Referral and Intake System (IRIS) and/or Nightengale NotesSkills:Customer serviceCommunicationProblem-solvingMaintain confidentialityOrganizationMulti-taskingTime managementDetail orientedRequired Documents:ResumeCover LetterComprehensive Benefits Package:Coverage begins on day one for health, dental, and vision insurance and includes health expense accounts with generous employer contributions if the employee participates in a qualifying health plan. One paid discretionary day is available after six months of employment, and paid time off for bereavement, jury duty, military service, and parental leave is available after 12 months of employment.
