Optum At Home is seeking a Registered Nurse to join the field-based care team delivering in-home visits and telephonic outreach as part of an interdisciplinary group. You will coordinate and monitor patient care, collaborate with PCPs and specialists, and support patients across care settings to promote health and prevent deterioration. You’ll operate with CI and CEU maintenance while complying with state rules. #J-18808-Ljbffr Optum
