Field-Based RN Case Manager — In-Home Care Coordination
Registered Nurse
Optum Home & Community Care is expanding its At Home program and seeking a Field Case Manager RN to deliver in-home patient care and coordinate services with an interdisciplinary care team. You’ll assess health needs, develop plans of care, and document in the EMR while supporting patients across the care continuum. Travel is required within a 60-mile radius of Columbus, OH, and you’ll partner with physicians, behavioral health specialists, and care navigators to promote health and prevent #J-18808-Ljbffr Registered Nurse
