UnitedHealth Group's Optum At Home program seeks an RN Field Case Manager to support patients in their homes. You will work with an interdisciplinary team, perform home visits, telephonic outreach, and plan care with the PCP and specialists. You will assess health needs, coordinate services, and educate patients while documenting in the EMR. Travel within 60 miles and maintaining licensure are required. #J-18808-Ljbffr UnitedHealth Group
