RN Case Manager – Senior Care Options (SCO)
Hybrid | 2 Days Remote + 3 Days Field/Office
Monday–Friday | 8:00 AM – 4:00 PM
No nights No weekends No on-call

What You'll Do:

Conduct in-home assessments and create personalized care plans
Manage a caseload of SCO members with complex needs
Coordinate care with providers, families, and community resources
Educate members and caregivers to support better outcomes
Ensure accurate documentation and compliance
Collaborate with a supportive, team-oriented care group

What You Bring:

Active RN license in Massachusetts
3+ years of nursing experience (5+ preferred)
Background in case management, home care, or community health (a plus)
Strong communication and organizational skills
Comfort with EMR systems and Microsoft Office
Reliable transportation for local travel
