Occupational Therapist - Full-time

Position Summary: Performs Occupational Therapy evaluation, reassessment and consultation; provides plan of care, discharge planning, patient, and significant other education; renders occupational therapy services within the practice guidelines of the American Association of Occupational Therapy (AOTA) and other regulatory agencies. Essential Duties and Responsibilities:

- Performs Occupational Therapy evaluation, reassessment and consultation upon referral of a physician, dentist or podiatrist.
- Provides plan of care and discharge planning. Secures a physician certificate for the plan of care.
- Provides Occupational Therapy services within the practice guidelines of the AOTA, the Illinois Occupational Therapy Practice Act and other regulatory agencies.
- Participates in discharge planning, when requested.
- Communicates with patient's physician, nurse and case manager when necessary regarding the patient's evaluation report, progress and discharge plan.
- Ensures patient safety, patient rights and HIPAA compliance.
- Bachelor of Science or Master of Science in Occupational Therapy.
- Occupational Therapy Licensure
- BLS Certification.
- OT Certification
