Registered Nurse (RN) - HospiceAs a Hospice RN, you provide end-of-life care focused on comfort, symptom management, and supportive documentation for patients and families.You work within interdisciplinary teams to honor patient goals and coordinate with community resources for home or facility-based care.ResponsibilitiesAssess symptoms, manage pain and comfort, and provide comprehensive nursing careAdminister medications and monitor adverse effects; adjust plans in collaboration with the teamCoordinate with physicians, social workers, chaplains, and home health aidesEducate families about advance directives, care planning, and bereavement supportDocument care plans and patient status in the EHR; ensure timely communicationSupport discharge planning and transitions to home or hospice facilitiesRespond to clinical changes and provide crisis management as neededQualificationsEducation: BSN preferred; ADN acceptable with hospice or palliative care experienceLicensure: Current, active Registered Nurse license in the United StatesCertifications:Basic Life Support (BLS)Palliative Care certification such as CHPN preferred but not requiredExperience or Skills: Experience in hospice or palliative care; strong communication, assessment, and family-support skillsExperienceExperience providing end-of-life nursing care in home or facility settingsExperience coordinating with interdisciplinary teams and community resourcesExperience documenting care plans and patient goals in the EHRPreferredCHPN or equivalent hospice certificationExperience with care transitions and bereavement supportWork EnvironmentHospice teams provide care in patients' homes or hospice facilities; interdisciplinary collaboration is essentialShifts may include days with on-call coverage; documentation and phone triage are common components
