Director, 340B Pharmacy Operations — Infusion
$140k - $180k Director, 340B Pharmacy Operations — Infusion Location: Remote + 20% travel
Compensation: $140K–$180K Base + 15% Bonus REQUIRED: Direct 340B experience in home infusion, outpatient infusion, or a specialty pharmacy with infusion operations. Candidates with 340B experience but no infusion background will not be considered. THE ROLE
Lead the implementation and execution of 340B programs across a growing infusion business. This is a hands-on role for someone who can take 340B initiatives from concept through implementation, go-live, and optimization — not simply oversee an established program. RESPONSIBILITIES
• Lead infusion-focused 340B implementations and launches
• Build project plans, workflows, timelines, testing, and go-live processes
• Optimize eligibility, accumulators, charge capture, billing, NDC mapping, purchasing, and replenishment
• Manage TPA/split-billing setup, data feeds, account configuration, and auditing
• Partner with infusion, pharmacy, health-system, finance, IT, wholesaler, and TPA teams
• Identify missed capture, operational gaps, compliance risks, and revenue opportunities
• Support HRSA/OPAIS requirements and ongoing compliance REQUIREMENTS
• Hands-on 340B + infusion experience required
• Must have home infusion, outpatient/health-system infusion, or specialty pharmacy with infusion operations:
• Experience personally implementing or materially changing 340B programs
• Working knowledge of HRSA/OPAIS and 340B compliance
• Strong knowledge of TPAs, split billing, accumulators, eligibility, charge capture,
billing, and drug purchasing WHY THIS ROLE
This is a build-and-scale opportunity with significant leadership exposure and long-term growth potential. The goal is for this person to develop into the leader who can independently run the 340B program as the infusion business grows.
