Linc Housing in Long Beach seeks an Intensive Case Manager to join the Intensive Case Management Services team. You will coordinate services for formerly homeless individuals with chronic medical conditions, partnering with VA, housing authorities and community providers to support housing stability. The role emphasizes motivational interviewing, harm reduction, trauma-informed care and a person-centered approach to help residents meet treatment goals and maintain housing. #J-18808-Ljbffr
