Physical Therapist (PT)Location: Phoenix, AZ Job Type: Full-Time Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM Patient Population: Primarily adults aged 70+ Daily Caseload: 10-12 patientsPosition SummaryWe are seeking a compassionate and dedicated Physical Therapist (PT) to join our healthcare team. In this role, you will evaluate, develop, and implement individualized treatment plans to help patients restore function, improve mobility, relieve pain, and prevent or minimize long-term physical disabilities resulting from injury, illness, or other medical conditions. You will work collaboratively with physicians, nursing staff, and interdisciplinary teams to deliver exceptional patient care.Key ResponsibilitiesEvaluate patients and develop individualized treatment plans based on physician referrals, patient needs, and functional goals.Administer physical therapy treatments and modify care plans according to patient progress and tolerance.Establish realistic, measurable, and achievable rehabilitation goals.Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate patient care.Monitor, document, and maintain accurate patient records in compliance with state, federal, and facility regulations.Educate patients and their families on treatment plans, home exercise programs, and preventive care.Conduct patient screenings and recommend appropriate therapeutic interventions.Communicate patient progress, concerns, and treatment recommendations with patients, families, and healthcare providers.Maintain professional standards while ensuring timely documentation and compliance with facility policies.Demonstrate punctuality, professionalism, and consistent attendance.QualificationsEducationBachelor's, Master's, or Doctor of Physical Therapy (DPT) degree from an accredited Physical Therapy program.Licensure & CertificationsCurrent Physical Therapist (PT) license in the state of employment.Current CPR/BLS Certification.Compensation & BenefitsMedical InsuranceDental InsuranceVision Insurance401(k) Retirement PlanSign-on AssistanceRelocation Assistance (if applicable)Paid Time OffAdditional company benefits
