Work Environment

This position is primarily onsite in our clients' homes within Matawan and the surrounding areas. The nature of home care means you'll encounter diverse environments, assessing and adapting your therapy approaches to fit each client's unique living situation. You'll be working with clients of all ages, from pediatric to geriatric populations, creating a dynamic and rewarding atmosphere that promotes healing and independence.

Key Responsibilities

As a Physical Therapist at Confluent Rehabilitation, your key responsibilities include:

• Directing comprehensive physical therapy treatment aimed at preventing disabilities and relieving pain, while promoting the development and restoration of muscle function per individualized care plans.
• Overseeing the work of physical therapy assistants, ensuring accurate implementation of treatment plans, and providing guidance during therapy sessions.
• Conducting thorough evaluations of clients' physical function using diagnostic tests related to muscle, nerve, joint, and functional abilities.
• Assessing home environments to identify enhancements for client independence, making informed recommendations when necessary.
• Delivering prescribed physical therapy services in client homes, including manual therapeutic exercises, supervision of non-manual exercises, and administration of traction and massage treatments.
• Evaluating and fitting prosthetic and orthotic devices, and making necessary modifications in conjunction with prescribing physicians.
• Participating in collaborative care plan development and updating processes, along with interdisciplinary team discussions to ensure comprehensive treatment plans are maintained.
• Recording and reporting any changes in clients' conditions to relevant parties, while preparing initial evaluations, care plans, progress notes, and discharge summaries.
• Providing education and support to clients and families regarding the use and maintenance of physical therapy devices and overall therapy programs.
• Supervising and mentoring Physical Therapy Assistants, ensuring compliance with state regulations.

Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate in Physical Therapy (DPT) is required.
• A valid PT license in the state of New Jersey is essential.
• Current CPR certification and a valid driver's license with automobile insurance are mandatory.

Preferred Experience
