RN Hospice Case Manager (Full-Time, Permanent Hire Position with Excellent Full Benefits, Competitive Compensation/Bonus Structure and much more!) Position Responsibilities: Coordinate and oversee all aspects of assigned patients' hospice care Develop and implement individualized plans of care with the interdisciplinary team Perform head-to-toe assessments, monitor symptoms, and manage medications Educate patients and families regarding condition, needs, and care Communicate patient progress with clinical leadership, physicians, and the care team Current Registered Nurse license. Current driver's license Current CPR certification Two years of Registered Nurse experience in a clinical care setting One year of full-time hospice experience Reliable transportation and valid automobile liability insurance Health Care Plan (Medical, Dental & Vision) Retirement Plan (401k, IRA) Life Insurance (Basic, Voluntary & AD&D) Paid Time Off (Vacation, Sick & Public Holidays) Short Term & Long Term Disability Training & Development #J-18808-Ljbffr Ethos
