RN - Case Management

Clinical Case Manager

Day shift with a 5x8 schedule.

This is a 13-week contract position.

The Clinical Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management, while also completing medical record documentation.

Responsibilities:

• Identify patient problems impacting their treatment plan and assist families in adjusting to healthcare situations.
• Support patient and family participation in the plan of care and safe discharge planning.
• Collaborate with an interdisciplinary team to develop clinical plans aimed at timely discharge to appropriate care levels.
• Conduct utilization reviews and ensure compliance with medical necessity review criteria.
• Manage community resource systems to assist patients and families in accessing necessary services.
• Maintain accurate medical documentation per policy and state regulations.
• Promote safety and adhere to guidelines related to patient and employee safety.

Required Experience / Certifications / Licensure:

• Current licensure in a clinical discipline (RN, RT, SW preferred) per state guidelines.
• Associates degree in a healthcare field (Bachelor's degree preferred).
• Working knowledge of the insurance industry and government reimbursement protocols.
• Two years of experience in Case Management and Discharge Planning in an Acute Care or Rehab setting.
