Virtual Kidney Care Navigator & Educator

Fresenius Medical Care is seeking a Kidney Care Specialist in Cleveland, OH to support patients with Chronic Kidney Disease and End Stage Renal Disease through virtual care navigation and education. The role requires collaboration with nephrologists and care teams, providing timely patient education and support. Ideal candidates should have a High School diploma and 1-3 years of experience in kidney care or chronic disease management.
