Overview
Physical Therapy Assistant, LTAC, Reading, PA. This travel assignment with Healthforce Healthcare LLC starts 09/07/2026 and runs for 13 weeks. Weekly pay ranges from $1,529 to $1,653. Responsibilities
- Implement and modify PT plans of care under supervision of a licensed PT in LTAC, delivering therapeutic exercises, gait training, ROM, balance activities, and use of assistive devices as prescribed.
- Monitor patient progress during and after sessions; document outcomes and communicate changes to the supervising PT and care team.
- Promote safe patient handling, adhere to infection control protocols, and maintain a clean, organized therapy area and equipment.
- Collaborate with nursing, OT, speech therapy, and other disciplines to optimize rehabilitation goals and discharge planning.
- Educate patients and families on home exercise programs, safety, and progression; provide clear instructions and support adherence.

Requirements
- Active Physical Therapy Assistant license in Pennsylvania (or eligibility to practice per state regulations).
- Graduation from an accredited PTA program (associate degree preferred).
- Prior LTAC or geriatric rehabilitation experience preferred.
- Strong communication and documentation skills; ability to work effectively within a multidisciplinary team.
- Availability for a 13-week travel assignment with a 5x8 shift schedule.
