As you engage directly with patients in their own environments, you'll have the opportunity to assess and adapt treatment strategies based on real-world conditions. Our collaborative home care model allows for a close-knit atmosphere with ongoing support from colleagues and supervisors, ensuring that you are never alone in your endeavors to enhance patient well-being.Key ResponsibilitiesConduct comprehensive assessments of patients' physical conditions and functional capabilities through direct observation and standardized evaluation tools.Create and implement tailored treatment plans that align with patients' rehabilitation goals, utilizing a variety of therapeutic interventions such as manual therapy and neuromuscular re-education.Deliver hands-on physical therapy through therapeutic exercises and modalities, while continuously motivating and educating patients to optimize adherence to their treatment schedules.Instruct patients and their families on effective home exercise programs, injury prevention tactics, and make recommendations for lifestyle modifications to further support rehabilitation progress.Regularly monitor, document, and assess patient improvements, adjusting treatment plans as necessary to ensure effective outcomes and adherence to best practices.Work collaboratively with a multidisciplinary team of healthcare professionals, including physicians and nursing staff to provide comprehensive care and to improve overall patient results.Maintain accurate and up-to-date patient medical records and documentation in accordance with health regulations and standards, utilizing EHR systems proficiently.Remain engaged with ongoing professional development through attendance at workshops and seminars, integrating new therapeutic techniques into practice.Patient Population / Client FocusOur client population includes a diverse range of individuals recovering from surgeries, injuries, and chronic conditions. Understanding the nuances of providing care within a home environment is crucial to helping these individuals regain their independence and improve their quality of life.QualificationsA Doctorate or Master's degree in Physical Therapy from an accredited institution.Possession of a current Physical Therapist license to practice in Florida.A minimum of three years of experience in physical therapy, particularly in settings providing direct patient care, is required.Strong clinical skills that include the capability to carry out and evaluate various therapeutic techniques effectively.Exceptional communication and interpersonal skills to effectively engage with patients, their families, and healthcare staff.The ability to work autonomously with integrity, as well as collaboratively within a multidisciplinary team environment.A profound commitment to delivering high-quality, patient-centered care at every interaction.Preferred ExperienceExperience in home care settings is highly advantageous, as it provides familiarity with unique patient dynamics.
