Job Description:\n\n The Director of Facility Partnerships and Clinical Support serves as NIVA Health’s primary clinical, relationship, and growth representative for assigned Skilled Nursing Facilities, Assisted Living Facilities, and Independent Living communities. A central responsibility of this position is establishing NIVA’s initial presence and first meaningful touchpoints within prospective and newly contracted facilities. The Director builds early trust with facility administrators, nursing leadership, clinical teams, and other key stakeholders; identifies facility-specific clinical and operational needs; and aligns NIVA’s wound care services with those needs. This position requires documented experience working directly within skilled care facilities and intimate knowledge of facility operations, clinical workflows, leadership structures, referral processes, quality programs, staffing challenges, and resident-care needs. The successful candidate must be able to assess how a facility operates, identify clinical and operational priorities, and explain how NIVA’s services can support wound care access, outcomes, staff effectiveness, continuity of care, and facility performance. The role combines initial facility engagement, clinical education, facility account management, partnership development, service implementation, and referral growth. Performance will be evaluated based on facility engagement, successful onboarding, relationship development, patient census, facility utilization, referral growth, partner retention, and measurable outcomes. Duties and ResponsibilitiesInitial Facility Engagement and Relationship DevelopmentServe as NIVA’s lead representative during the initial stages of facility outreach, partnership development, and implementation.Establish the first meaningful relationships with prospective and newly contracted facility leaders.Conduct initial on-site meetings with Facility Administrators, Executive Directors, Directors of Nursing, Assistant Directors of Nursing, Unit Managers, wound care nurses, quality and compliance leaders, social services, discharge-planning personnel, therapy leadership, and other stakeholders involved in resident care.Assess each facility’s wound care structure, patient population, staffing model, clinical resources, referral processes, documentation practices, quality priorities, and operational needs.Identify gaps in wound care access, clinical oversight, treatment continuity, staff education, specialty-provider availability, and care coordination.Align NIVA’s clinical services, provider model, rounding structure, communication practices, and educational resources with the specific needs of the facility.Clearly define NIVA’s role, facility responsibilities, referral expectations, communication pathways, and implementation requirements during initial meetings.Build early trust by demonstrating clinical credibility, operational awareness, responsiveness, and detailed knowledge of skilled care facility processes.Ensure facility leadership and frontline teams understand how and when to engage NIVA.Establish an appropriate communication and relationship cadence before clinical services begin.Maintain continuity from the initial facility meeting through onboarding, implementation, and ongoing account management.Facility Relationship ManagementServe as the primary point of contact for assigned Skilled Nursing, Assisted Living, and Independent Living facility partners.Develop trusted relationships with facility administrators, executive directors, nursing leaders, clinical teams, social services, therapy departments, and other appropriate stakeholders.Conduct weekly on-site visits to assigned facilities to maintain strong operational, clinical, and leadership relationships.Maintain a consistent cadence of in-person, telephone, email, and virtual communication.Attend facility meetings as appropriate, including Quality Assurance and Performance Improvement meetings, Resident Council meetings, care coordination meetings, interdisciplinary clinical meetings, Administrator or Executive Director meetings, Director of Nursing leadership meetings, and discharge-planning meetings.Maintain a professional presence that reflects NIVA’s clinical excellence, responsiveness, and partnership-focused service model.Position NIVA as a trusted clinical and operational partner rather than solely as an outside service vendor.Develop a working understanding of each facility’s priorities, staffing challenges, quality goals, census considerations, and resident-care needs.Facility Partnership DevelopmentDevelop and execute territory-specific growth strategies focused exclusively on Skilled Nursing, Assisted Living, and Independent Living facilities.Identify prospective facility partners that would benefit from NIVA’s mobile wound care services.Conduct on-site introductory meetings, clinical presentations, leadership discussions, educational sessions, and service evaluations.Build relationships with facility decision-makers and clinical influencers.Present NIVA’s clinical model in a manner that is relevant to each facility’s operational and resident-care needs.Clearly communicate NIVA’s clinical capabilities, provider model, treatment services, facility rounding process, communication standards, documentation expectations, patient eligibility requirements, clinical outcomes, and facility-support resources.Identify opportunities to expand services into additional units, buildings, sister facilities, and affiliated communities.Achieve assigned facility-development, patient census, admission, referral, and territory-growth expectations.Maintain a consistent prospecting and relationship-development cadence throughout the assigned territory.Coordinate with sales leadership on partnership proposals, service agreements, and implementation planning.Facility Onboarding and Clinical IntegrationLead or coordinate initial onboarding meetings with facility administration, nursing leadership, clinical staff, and ancillary departments.Develop a facility-specific implementation plan based on the organization’s workflows, staffing model, resident needs, and operational structure.Educate facility staff regarding NIVA’s clinical service model, appropriate patient identification, referral and intake procedures, patient eligibility requirements, scheduling and rounding workflows, documentation expectations, communication pathways, clinical escalation processes, treatment capabilities, and facility and NIVA responsibilities.Support facility leadership and clinical staff in establishing appropriate resident-identification, referral, communication, and implementation workflows.Confirm that key facility personnel understand how referrals are initiated and how communication will occur after services begin.Coordinate with NIVA’s clinical, intake, case management, scheduling, credentialing, and operational teams to support a seamless transition into patient care.Remain actively involved during the initial weeks of service to identify and correct implementation barriers.Reinforce agreed-upon workflows when referral, scheduling, documentation, or communication processes are not being followed.Evaluate early utilization and determine whether additional education, leadership engagement, or workflow modification is needed.Clinical Education and Facility SupportServe as a clinically credible resource during facility presentations, partnership discussions, onboarding sessions, account reviews, and educational programs.Address clinical and operational questions from administrators, executive directors, nursing leaders, facility clinicians, and other stakeholders.Translate complex wound care concepts, clinical protocols, and treatment options into clear and approachable information.Conduct clinical in-services and educational programs for facility staff.Develop and refine clinical marketing materials, facility training guides, implementation resources, referral guides, educational presentations, and facility-facing clinical content.Provide education regarding wound identification, escalation, referral criteria, available treatment options, and the appropriate use of NIVA’s services.Educate internal teams regarding facility workflows, skilled care operations, and the clinical value proposition of NIVA’s facility-based services.Represent NIVA as a clinical resource at conferences, trade shows, professional meetings, and industry events.Patient Census and Facility UtilizationMonitor active patient census, new admissions, discharges, treatment volume, and provider utilization by facility.Evaluate whether eligible residents are being consistently identified and referred.Identify census declines, underutilization, referral gaps, delays in patient identification, workflow barriers, communication breakdowns, and staff education needs.Develop and implement facility-specific interventions, including staff education, leadership engagement, referral-process reinforcement, workflow modification, patient-identification initiatives, additional rounding support, and expansion to additional units.Partner with clinical, intake, scheduling, and case management teams to maintain appropriate patient access and scheduling efficiency.Ensure facility utilization reflects the organization’s clinical needs and the established scope of the partnership.Proactively engage facility leadership when patient volume falls below expectations.Account Growth and Partner RetentionMaintain ongoing engagement following facility implementation.Reinforce NIVA’s value proposition with facility leadership and frontline staff.Conduct routine account reviews, leadership check-ins, and educational refreshers.Identify opportunities to increase facility utilization and expand the partnership.Solicit feedback regarding provider performance, clinical communication, scheduling, documentation, staff education, resident outcomes, responsiveness, and overall service satisfaction.Identify potential relationship or retention risks and promptly escalate significant concerns.Develop corrective action plans in collaboration with internal teams.Support facility retention, contract renewal, census growth, and long-term partnership development.Issue Resolution and Facility AdvocacyServe as the initial escalation point for assigned facility partners regarding scheduling concerns, documentation questions, provider communication needs, referral-processing challenges, operational concerns, service-delivery issues, and communication gaps.Evaluate concerns within the context of facility workflows and resident-care requirements.Coordinate with appropriate NIVA departments to resolve concerns promptly and professionally.Maintain clear ownership of facility communication to reduce confusion and prevent conflicting information.Communicate resolution plans, responsibilities, and follow-up expectations to facility leadership.Document significant concerns, interventions, and outcomes through designated company reporting processes.Proactively identify and address issues before they negatively affect resident care, facility utilization, or the long-term partnership.Performance Reporting and Metrics OversightProduce and present monthly facility performance reports that may include active patient census, new patient admissions, referral activity, treatment volume, patient discharges, wound healing outcomes, hospitalization and transfer trends, provider utilization, scheduling performance, facility engagement activity, and partner satisfaction.Present performance findings to internal leadership and appropriate facility stakeholders.Use data to identify account strengths, clinical needs, operational risks, and growth opportunities.Develop facility-specific recommendations based on performance trends.Participate in regular performance reviews with sales and operational leadership.Compliance and Administrative ResponsibilitiesMaintain compliance with HIPAA, applicable Medicare and Medicaid requirements, company policies, and healthcare regulations.Protect the confidentiality of resident, facility, and company information.Maintain accurate records of facility visits, presentations, onboarding activity, account concerns, educational programs, and follow-up actions.Manage approved marketing and travel expenses responsibly and within established budget expectations.Participate in required education, compliance programs, departmental meetings, and clinical training.Complete additional responsibilities as assigned to support facility growth, clinical excellence, and partner success Requirements Required QualificationsClinical background as an Licensed Nursing Home Administrator, NP, PA, therapist, or other qualified healthcare professional.Documented professional experience working directly within Skilled Nursing Facilities or skilled care environments.Demonstrated intimate knowledge of Skilled Nursing Facility operations, including nursing leadership structures, clinical workflows, resident-care processes, interdisciplinary care coordination, quality assurance and QAPI processes, patient identification and referral practices, documentation expectations, facility staffing structures, and regulatory and survey considerations.Demonstrated ability to assess facility-specific needs and align clinical services with operational and resident-care priorities.Strong understanding of wound care, chronic disease management, specialty clinical services, or facility-based care delivery.Experience presenting clinical information to both clinical and non-clinical audiences.Experience leading facility education, clinical in-services, onboarding, or healthcare-service implementation.Demonstrated ability to establish credibility with facility administrators, Directors of Nursing, clinical leaders, and frontline staff.Excellent written, verbal, interpersonal, and presentation skills.Strong organizational skills, professional judgment, and consistent follow-through.Ability to independently manage a multi-facility territory.Ability to interpret facility performance data and communicate actionable recommendations.Willingness and ability to travel frequently throughout the assigned territory.Valid driver’s license, reliable transportation, and active automobile insurance.Preferred Education and
