Travel RN - Home Health

American Traveler is seeking an experienced Home Health RN for a 13-week travel contract in California, requiring a valid CA RN license, BLS certification, and a valid driver's license with insurance.

Job Details

• Home health care setting, providing in-home nursing visits to recently discharged patients at high risk for rehospitalization,
• Primary patient population includes adults with CHF, COPD, wounds (including wound vacs), and IV/PICC line management; no pediatric patients,
• Average caseload of approximately 5 visits per day, based on a point system (start of care: 2.5 pts, recertification: 1.5 pts, resumption of care: 1.75 pts, routine visit: 1 pt), totaling 5.25 points per day,
• Charting performed in Cerner EMR,
• 8-hour day shifts, 5 days per week; schedule runs either Monday-Friday or Tuesday-Saturday (Tuesday-Saturday preferred),
• Every other weekend (EOW) required,
• Holiday coverage needed, particularly for Thanksgiving and/or Christmas,
• No on-call or standby requirements,
• Floating within home health only,

Job Requirements

• Active CA RN license, fully active at time of consideration - pending licenses not accepted,
• Active BLS certification required - pending certifications not accepted,
• Valid driver's license and personal auto insurance required,
• Minimum 2 years of RN experience required,
• Experience with HomeCare HomeBase (HCHB) preferred,
• 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire,
• Driver's license required for consideration,

Additional Information

• Conduct in-home nursing assessments, start-of-care visits, recertifications, resumption-of-care visits, and routine visits,
• Patient education related to respiratory equipment (CPAP, BiPap, vents) is performed as part of standard care,
• A home health aide is part of the department team,
• Supervisor (Holly Haynes) is available during working hours for routine questions and after hours for urgent needs; an administrator on call is also available after hours and on weekends,
• Schedule is typically set by the previous day, but morning changes are common due to unprocessed start-of-care orders or sick calls,
• Any work-appropriate scrubs are acceptable,
• Candidates must not have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity-affiliated facility within the past year; candidates employed PRN through outside staffing firms will be considered,
• RNs at CommonSpirit Sacramento-region facilities must pass the Relias Dysrhythmia exam (basic or advanced) prior to an offer being confirmed,
• A pass/fail sterile technique test for Foley catheter insertion and central line dressing changes is administered on the first day of orientation
