Responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Carle Home Services specific patient populations and families throughout the geographical area. Hours may vary depending upon census and program need. Qualifications Certifications: Proof of Auto Insurance - Varies; Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) Instructor within 30 days - American Heart Association (AHA); Driver's License - Illinois Secretary of State (ILSOS). Education: College Diploma: Nursing. Work Experience: 1yr of Nursing experience. Responsibilities Demonstrates understanding of Hospice Regulatory requirements/benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory requirements regarding documentation standards. Comprehends department specific indicators. Performs case management activities of assessment, coordination, planning, monitoring, implementation, and evaluation. Interacts with clients, caregivers and families to assess, plan care, arrange services, monitor, and provide support and education. Completes all admission documentation within policy guidelines and submits timely. Ensures timely physician orders, corrections to care plans, and verbal orders per policy. Reviews orders with MD office, confirms read‑back, and transmits within required timeframe. Ensures patients visit records are updated and maintained. Completes Home Health Aide supervisory visits. Acts as coordinator of the health care team to maintain linkage within continuum of care. Collaboration: initiates case conferencing and documents interactions with other disciplines, MD, care coordinator, insurers. Communicates pertinent information for patient care conferences. Provides care according to plan of care and orders, sensitive to patients and families. Directs activities of Licensed Practical Nurse and supervises aides. Assesses patients for appropriate hospice placement per Medicare guidelines. Conducts home visits to monitor signs and symptoms and provides palliation interventions. Triage phone calls when on call. Performs post‑mortem care and notifies relevant parties upon patient passing. Provides comfort for family and makes referrals to Bereavement Coordinator. Compensation and Benefits The compensation range for this position is $33.62 per hour - $57.83 per hour. The actual compensation offered will be dependent on a variety of factors including experience, qualifications, location, training, licenses, shifts worked and compensation model.
