Compensation + Benefits

• Base Salary: $75,000-$85,000 (depending on experience and license level)

What You'll Do

• Clinical care: Conduct comprehensive psychosocial assessments and mental health screenings, build individualized care coordination and intervention plans, and provide short-term counseling, crisis intervention, and psychoeducation to keep patients stable, safe, and supported.
• Patient experience & coordination: Deliver compassionate, patient-centered care, run HIPAA-compliant telemedicine consults, and manage a digital patient load with timely resource connection and follow-ups alongside primary care, urgent care, and aftercare teams.
• Tech-enabled documentation: Document encounters, assessments, and referrals in the EMR, using AI copilots for documentation, resource sourcing, and admin, keeping records timely and compliant with New York standards.
• Team collaboration: Partner with clinical leads, care coordinators, and the medical team (MDs, NPs, PAs), advising on community resources, insurance barriers, and psychosocial factors, and contribute ideas to improve clinic workflows across our hubs.
• Flexibility & growth: Join monthly syncs and one-on-ones, complete ongoing training on new community resources and social work practices, and stay flexible with scheduling, including some evenings or weekends.

Requirements

• Education: Master's degree in Social Work (MSW) from an accredited institution.
• Licensure: Active New York State license (LMSW or LCSW) in good standing.
• EMR & compliance: EMR experience and strong HIPAA knowledge.
• Skills: Excellent communication, active listening, and crisis de-escalation.
• Preferred: 1+ year healthcare or outpatient clinic experience; bilingual English/Spanish.
• New grads welcome: Must hold active NYS licensure.
