Location: Avera McKennan Hospital Worker Type: Regular Work Shift: Day/Evening/Weekend/Holiday Shift (United States of America) Pay Range: $32.50 - $48.50 Position Highlights May be eligible for a $15,000 sign on bonus. 1-3 years working in a clinical environment. 8am-8:30pm shift with weekend and holiday coverage (every 4th weekend, 2-3 per year), minimal call time. A Brief Overview The Registered Nurse (RN) – Hospital at Home provides acute-level nursing care to patients receiving hospital services in their homes. The RN delivers comprehensive, evidence‑based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The RN functions as part of an interdisciplinary care team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model. What you will do Provide patient care by reviewing physician prescriptions and communicating the nursing care plan to other members of the health care team. Coordinate with other healthcare providers and provide physical, emotional, psychological, and spiritual support to patients, families and friends. Facilitate interventional care in the home utilizing clinical expertise and problem‑solving to make recommendations to physicians based on comprehensive assessments. Develop nursing care plans by obtaining medical histories, examining patients, documenting patient condition, psychological status, and influencing social factors. Draw labs; provide respiratory care; confer with family and friends; develop diagnoses and establish goals, interventions, schedules, and timetables. Teach healthcare requirements by establishing patient care goals and instructing patients, families, and friends on condition, medications, and self‑care skills. Evaluate patient care by adhering to therapeutic standards; measure outcomes against standards; review records; note changing conditions; modify care plans as needed. Maintain a safe, secure, and healthy care environment by adhering to infection‑control policies, medication administration and storage procedures, controlled substance regulations, and legal regulations. Document patient care services by charting in patient and department records. Keep patient care equipment operating by following procedures, troubleshooting, and maintaining supplies. Keep supplies ready by checking stock, anticipating needs, expediting orders, and verifying receipt. Maintain competence in nursing capabilities as outlined by leadership. Essential Qualifications Work the hours specified; perform essential job functions satisfactorily. Have visual acuity adequate for duties; communicate effectively; hear, understand, and distinguish speech and other sounds. Registered Nurse (RN) – Board of Nursing licensure in the state of practice. Preferred: 1-3 years of Med Surg, ER, or home health experience.
