Acute Care Case Manager – Discharge Planning
Schedule
Weekend requirement: Every other weekend; must be available for holidays. Holiday commitment: Must commit to working 3 of the following 5 — Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day.
Assignment Details
Assignment type: Acute care hospital. Daily caseload: up to 25 patients. Orientation: 40 hours.
Requirements
- California state license required; must be active by start.
- 2 years of acute care case management or discharge planning experience required.
- Travel experience required; first-time travelers may be considered after screening.
- InterQual experience highly preferred.
- Key clinical skills include care coordination, discharge planning, utilization review, and knowledge of Medicare/MediCal and payer systems.
- Experience with advance directives and behavioral/mental health crisis navigation preferred.
- Charting system: Epic.
Responsibilities
- Coordinate patient care and discharge planning activities.
- Conduct utilization reviews and determine medical necessity.
- Arrange durable medical equipment, skilled nursing facility placements, and ambulance transport as needed.
- Navigating aspects of protective services and behavioral health crises.
- Provide documentation in computerized systems.
- Collaborate with interdisciplinary teams to ensure comprehensive patient support.
Compliance / Notes
Pre-approved time off limited to 1 request per contract; maximum of 7 days. Background checks required.
