Licensed Practical Nurse (LPN)The Licensed Practical Nurse (LPN) performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures for our beautiful Assisted Living/Memory Care building (Meadow Woods) in Bloomington, MN.The schedule includes:72 hours in two weeksDay shiftEvery 4th weekendAs part of the Martin Luther Campus Senior Community, Meadow Woods Assisted Living is a residential setting by woodlands, wetlands, mall of america, and the Minnesota River Bluffs. Our 113 unit assisted, independent and memory care buildings are located in East Bloomington, MN near public transportation. Our staff encourages participation in the many regularly scheduled activities and lifelong learning programs. Managed by Ebenezer we believe their mission to heal, discover, and educate for longer healthier, more meaningful lives.ResponsibilitiesProvides professional nursing care:Consults with Registered Nurse (RN) regarding resident condition and treatments.Promptly transcribes and implements practitioners' orders.Orders medication and supplies in a timely mannerAssists RN in adherence to medication administration procedures.Assembles and operates medical equipment used for resident care.Reports changes in resident status to the RN.Initiates emergency treatment as needed. Reports status to RN.Performs skilled nursing care per individualized care plan under the direction of RN.Carries out proper charting practices and proceduresAssists in completing supervisory/monitoring visits.Performs any other duties as assigned or delegated by RN.Maintains current observation and documentation of tenant/resident health status.Supports RN to complete initial and on-going assessments per Minnesota home care regulations and Nurse Practice Act.Assists RN to assure that tenants/residents have up-to-date service agreements and care plans.Maintains clinical charts and progress notes using computerized documentation system.Communicates changes in resident conditions to RN, practitioners, and other relevant individuals.Required QualificationsLess than 1 year experienceMN Licensed Practical Nurse (LPN)Basic Life Support (American Heart Assoc or Red Cross)Preferred QualificationsAssociate of Science LPN1 year experienceBenefit OverviewEbenezer offers a generous benefits package, including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
