Travel Physical Therapist position in North Dallas (75240) - Home Health & Hospice travel contract with competitive pay, mileage reimbursement, and a supportive clinical team. Job Details
- Location: Dallas, TX 75240 (Home Health / Hospice, field-based) - North Dallas service area
- Employment Type: Travel Contract / Travel PT
- Contract Duration: 13 weeks (April 13, 2026 - July 13, 2026)
- Schedule: Monday - Friday, 8:00 AM - 4:30 PM
- Guaranteed Hours: 40 hours/week
- Shift Duration: 8-hour shifts
- Weekend Requirement: 1 weekend per month (on-call)
- Estimated Weekly Pay: $2,030 - $2,190 (competitive travel PT pay)
- Mileage: Reimbursed between patient visits
- Dress Code: Navy scrubs
- Active and valid Physical Therapist (PT) license - eligible to practice in Texas
- Master's degree or higher in Physical Therapy (DPT preferred)
- Minimum 1 year of experience as a Physical Therapist; 2 years home health experience preferred
- Experience with Homecare Homebase (HCHB) documentation system
- Ability to supervise and collaborate with Physical Therapist Assistants (PTAs)
- Reliable transportation for travel within the Dallas service area
- Provide high-quality home health physical therapy and hospice physical therapy to patients across Dallas, delivering patient-centered and compassionate care
- Complete 30 productivity points per week, including OASIS assessments and treatment visits
- Submit clear and timely documentation within 24 hours of each visit using HCHB
- Attend weekly case conferences (Thursdays at 9 AM) and collaborate with interdisciplinary teams
- Supervise, mentor, and support Physical Therapist Assistants (PTAs) during field visits
- Manage caseloads across diverse, high-acuity home health and hospice patients while maintaining safety and clinical best practices
