Licensed Practical Nurse

Duties include but are not limited to: The primary base location for this position will be located at Shavano Park Outpatient Clinic at 4350 Lockhill Selma Rd, Suite #200, San Antonio, Texas 78249. As an integral part of the Patient Aligned Care Team (PACT) LVN, you will demonstrate sound techniques in complex nursing care procedures and basic care of the patient in the Primary Care Clinic setting. You will provide nursing care in a self-directed manner with work accomplished requiring minimal review by the RN in charge and is administratively responsible to the Nurse Manager or designee for the quality and quantity of work performed daily and is responsible and accountable for his/her individual practice. You will complete a focused patient intake and document appropriately. You must possess excellent communications, interpersonal, customer service skills. In addition to providing clinical support, you will be responsible for actively participating in staff meetings and/or workgroups, clean and prepare exam rooms, assist the physicians with procedures and close open encounters, recognize and appropriately respond to breakage/malfunction or loss of equipment.

GS-3:

• Performs assigned LVN duties under close supervision.
• Performs routine structured/focused health screenings or the equivalent, delivering basic nursing interventions, promoting patient safety, and assisting with ensuring smooth clinic flow, while reinforcing patient education, preventive care, and continuity of services across the care experience

• Performs assigned LVN duties under frequent guidance.
• Performs preventive procedures, nursing tasks, chart reviews, panel scrubbing, daily huddles, and visit preparation in accordance with established policies, procedures, and techniques.

• Carries out complex practical nursing care assignments in Outpatient Primary Care.
• Provides a full range of practical nursing care and patient-centered support by confirming the reason for visit, conducting pre-visit outreach, reinforcing instructions, and ensuring Veterans bring needed logs, equipment, and health information.

• Carries out the most complex practical nursing care assignments in Outpatient Primary Care.
• Uses judgment in selecting the appropriate order and sequence of procedures and treatments; supports clinical decision-making by reviewing medications and allergies, identifying urgent needs, initiating nursing protocols or clinical pathways as locally approved, and communicating key findings to the RN or provider.

Work Schedule: Monday - Friday, 7:30am - 4:00pm, or 8:00am - 4:30pm. Must be flexible other Tours maybe added.

Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
