Palliative Nurse Practitioner

Role Overview The Nurse Practitioner will work in accordance with prescribed policies and procedures of the organization, including all applicable state and federal regulations and under the general supervision in conjunction with the Hospice Medical Director. The Advance Practice Nurse is responsible for performing patient medical examinations and palliative care visits relative to the face‑to‑face encounters required to certify patient eligibility for the Medicare Hospice Benefit after six months of hospice care. Documents visits in the patient’s clinical record and completes required face‑to‑face encounter forms. Provides medical care services and palliative care intervention for hospice patients on an as‑needed basis. Communicates effectively with the Medical Director, Administrator, and the Hospice Interdisciplinary Team. Responsibilities Perform patient medical examinations and palliative care visits. Basic Qualifications Graduate from a master’s‑level Nurse Practitioner or Clinical Nurse Specialist program. Completes a graduate level 3‑credit course in Pharmacology from an accredited program or a 45‑hour pharmacology course fully integrated into the Master’s in Nursing program. National certification – successfully pass the required examination. Currently licensed and nationally certified via the New Jersey Board of Nursing as an Advance Practice Nurse in New Jersey (N.J.A.C. 13:37‑7.1). Valid driver’s license and reliable insured transportation in accordance with New Jersey requirements. Skills Provide services in a timely and professional manner in accordance with operational policies and procedures. Adhere to Hospice’s compliance program, billing submission policies, Medicare Conditions of Participation, New Jersey Licensure Guidelines, and applicable laws. Demonstrate a positive, professional manner, manage time efficiently, and act as a role model. Develop and maintain effective positive relationships with the interdisciplinary group, other professionals, and physicians in the community. Maintain a compassionate, sympathetic attitude toward caring for hospice patients. Communicate effectively and courteously with patients, caregivers, colleagues, supervisors, coworkers, and others. Demonstrate optimism, enthusiasm, and willingness to work constructively with other health care team members and reflect concern for the wellbeing of patients and employees. Experience Minimum of two years’ experience as a Nurse Practitioner. Understanding of the hospice program of care. Compensation The Nurse Practitioner is paid on a per client visit rate at $125.00.
