Responsibilities The Nurse Practitioner (NP) acts as part of the clinical operations team and is responsible for providing direct patient care in ChenMed/Jencare medical centers, nursing homes, skilled nursing facilities (SNF), and home settings. The primary duties include geriatric assessment, medical history, physical exam, diagnosis and treatment, development of the nursing plan of care, health education, physician referrals, case‑management referrals, follow‑up, and clear documentation according to ChenMed standards for quality, service, productivity, and teamwork. The role also involves participation in clinical rounds and conferences, in‑depth documentation through written progress notes and summaries, risk and quality management programs, and patient‑care rounds. The NP must demonstrate the ability to function independently and collaboratively with other health‑care professionals, consult with the manager, physician, and medical director, adhere to departmental goals and regulatory compliance, and practice according to a written or electronic practice agreement. Primary duties include independent assessment of acute and non‑acute clinical problems, performing and documenting physical assessments and patient histories, analyzing trends, developing and implementing patient management plans, writing admission, transfer, and discharge orders, ordering and interpreting laboratory and diagnostic studies, prescribing medications, referring for consultations, performing invasive procedures upon competency, participating in rounds and conferences, communicating strategies to the team, collaborating with multidisciplinary members, recognizing critical situations requiring physician intervention, and initiating life‑saving procedures when necessary. The NP also collaborates with team members to analyze and improve health‑care delivery systems, participates in professional development activities, initiates arrangements and writes orders for SNF discharges, works collaboratively with physicians, nurses, PT, social workers, family, and caregivers to transition patients to lower levels of care, and facilitates discharge planning. Knowledge, Skills, and Abilities Demonstrated record of consistently achieving clinical performance metrics, strong critical thinking, problem‑solving skills, and the ability to communicate complex situations effectively. Education and Experience ARNP or similar advanced degree in Nursing required Current Nurse Practitioner Certification in the State of practice required Board certification by AANP or ANCC is preferred but may be required for certain States Current DEA number for schedule II‑V controlled substances may be required based on State of practice Minimum of 1 year of clinical experience preferred; strong new graduates may be considered Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross required within the first 90 days of employment ChenMed is changing lives for the people we serve and the people we hire.
