Minimum Requirements

Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
Active California PT license required to start the assignment.
BLS certification may be required from AHA or ARC.
Minimum 1 year of home health experience at a Medicare-certified agency. (Client-specific requirement.)
Experience with HCHB electronic medical record required. (Client-specific requirement.)
Productivity familiarity with treating approximately 6–7 patients per day preferred.

Facility Type: Home Health.
Shift Name: Days.
Shift Details: Monday–Friday, 8:00 AM–5:00 PM.
Schedule: Full-time, 40 hours per week.
On-call or weekend requirements: No.
Territory: Napa County, North. Candidate may be expected to travel within the assigned county.
Duration: 13 weeks.
