CNA - LTC/SNF

Job Description

Job Summary:
Provide compassionate CNA care to residents on a neurobehavioral unit with diagnoses of Alzheimer's and dementia, focusing on behavioral stabilization and safety. Support residents through direct assistance and clinical tasks in a structured environment.

Responsibilities:

Assist residents with activities of daily living, including ambulation, feeding, turning, and repositioning.
Perform clinical tasks such as bed baths, oral and perineal care, and skin care to maintain resident hygiene.
Monitor and record vital signs, weight, and urine output, ensuring accurate documentation.
Utilize skills in safety and mobility aids, including hospital beds, transfer belts, and wheelchairs.
Follow protocols for isolation precautions, decubitus care prevention, and routine catheter care.
Support behavioral management strategies and assist in de-escalating aggressive behaviors.

Qualifications:

Hold a valid NY State CNA license in hand.
Possess at least 1 year of prior experience in LTC or SNF settings.
Demonstrate proficiency in using EPIC charting software.
Be able to pass background checks and meet facility safety policies.
Adapt to floating shifts and flexible schedules, including evening hours.
Capable of performing physical tasks involving patient transfers and repositioning safely.
