Caretenders Hospice, part of LHC Group and the Optum family, is seeking a Registered Nurse Case Manager. You will oversee the Plan of Care for hospice patients, collaborate with the care team, and ensure compassionate, high-quality nursing care in patients' homes. Requires Ohio RN license, 1+ year clinical hospice experience, and a valid driver’s license. Travel within a 50-mile radius, PRN scheduling with on-call evenings/weekends; comprehensive benefits and 401k offered. #J-18808-Ljbffr Optum
