Responsibilities
- Provide Physical Therapy Assistant services under supervision to residents within the skilled nursing facility setting.
- Assist in evaluating mobility, balance, gait, ROM, and functional tasks; contribute to patient-centered treatment plans.
- Implement and adjust treatment plans, therapeutic exercises, and modalities as directed by the supervising PT.
- Document patient progress and treatment in the NetHealth (Rehab Optima) system; ensure accuracy and legibility of records.
- Monitor patient responses to interventions; communicate changes and plan of care to the supervising PT and care team.
- Maintain safe patient handling, proper body mechanics, and equipment safety; participate in discharge planning and home exercise program guidance.

Requirements
- Active California Physical Therapy Assistant license
- SNF experience and NetHealth (Rehab Optima) knowledge preferred
- Ability to work in a Skilled Nursing Facility setting in Monterey, CA
- Travel-ready with day-shift availability
- Comfortable collaborating with a multidisciplinary care team
