Palliative Care Nurse Practitioner

Gentiva Health Services, Inc.

Heartland Hospice is seeking a Nurse Practitioner to provide dual hospice and palliative care across the care continuum. The NP will lead clinical management, coordinate with IDG/IDT, and guide families through complex decisions with empathy.

Responsibilities include performing comprehensive assessments, managing visits, and ensuring compliance with Medicare/Medicaid requirements. A strong emphasis on teamwork, documentation, and quality of life is expected.
