Trauma-Informed Community Health Advocate

Inova Fairfax Hospital in Falls Church, VA is seeking a dedicated Community Health Worker to join our team on a full-time day shift. You will provide trauma-informed advocacy, care coordination, and connection to community resources to patients with complex needs. The role includes home visits, liaison with social services, and collaboration with hospital care teams to improve health outcomes and equity.
