Job Opportunity: Registered Nurse - Homecare Hospice

We are looking for a dedicated and empathetic Registered Nurse to join our Homecare Hospice team, serving the community in Connecticut. This position is primarily field-based, focusing on delivering skilled, patient-centered care for individuals at the end of life.

Key Responsibilities

• Conduct thorough assessments to evaluate patient needs.
• Manage pain and symptoms effectively to enhance patient comfort.
• Collaborate with an interdisciplinary team to develop personalized care plans.
• Provide education and support to families, promoting informed decision-making and dignity.
• Document all care activities in the electronic health record (EHR) system.
• Communicate any changes in patient condition promptly.
• Participate in on-call rotations as required.

Qualifications and Requirements

• Current Registered Nurse license in the state of Connecticut.
• Experience in hospice or palliative care is preferred.
• Strong communication and interpersonal skills.
