Hospice Admissions RN: Compassionate, Impactful Care
Midland Care
Midland Care Connection is a nonprofit, community-based organization in Topeka, Kansas. We provide hospice, home health, grief counseling, and PACE services to help older adults stay independent at home while receiving compassionate care. The Hospice Admissions Nurse will perform initial assessments, determine eligibility for hospice services, and explain care plans to patients and families.
