Physical Therapist

Schedule: M-F, Days; Rotating weekend coverage

Assignment Details: Guaranteed Hours: 40 Contract Length: 13 weeks

Requirements: Current Professional Licensure Required Current IL Physical Therapist License Required Valid Driver's license and proof of personal auto insurance Required BLS Required APTA Membership Preferred Physical Therapy minimum 1 year Preferred Strong computer skills including Microsoft Word, Excel and Outlook

Roles & Responsibilities: Evaluate, plan, direct, and administer Physical Therapy services as prescribed by a licensed Provider. Treat patients using Physical Therapy modalities and skilled therapy techniques; instruct patients on the use of adaptive/assistive equipment. Supervise and direct Physical Therapy Assistants; maintain open communication with members of the Rehabilitation team. Perform appropriate assessments on all patients; interpret results within scope of licensure. Design treatment programs for patients with various disabilities and medical problems. Formulate realistic short and long-term treatment goals in collaboration with the patient and family. Ensure accurate, timely documentation and compliance with regulatory standards.

Compliance / Notes: Comply with all established safety procedures to ensure a safe environment for patients, visitors, and staff. Participate in performance improvement activities.
