Overview Physical Therapist – Home Health Services. Location: Horry County. Full Time position. New Pay Per Point Model – Top pay in the industry. Clinicians are compensated based on the work performed and time spent outside of a visit. Summary: To provide evaluative, consultative and treatment interventions to home health patients in their place of residence referred by a licensed physician, while maintaining effective communication with home health partners, patients, families, and other internal and external customers as appropriate. Responsibilities Evaluate the patient’s level of function and skilled therapy needs. Treat the patient to relieve pain, develop or restore function, and achieve maximum outcomes according to the established plan of care. Advise and consult the family and other interdisciplinary team members to promote optimal functional outcomes. Assess, monitor, document, and report progress of the patient\u2019s health and condition using required documents via clinician preferred technology, within established timelines. Maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient plan of care, making referrals to other disciplines as needed. Demonstrate intermediate knowledge of home care and competency in discipline-specific patient care skills. Conduct field visits to evaluate and initiate start of care, and to conduct clinical outcomes monitoring, follow-up and care performance improvement initiatives. Utilize approved teaching materials in patient/family education. Complete documentation timely and accurately at the point of care, according to industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABNs, subsequent visits, physician orders, care coordination). Meet agency productivity standards and use time and resources effectively and efficiently. Participate in center/agency surveys, mandatory in-services, case conferences, staff meetings, and programs as required. Follow established safety regulations and report job-related functions/tasks involving occupational hazards as necessary. Maintain current license, certification, and clinical records to meet federal, state, and agency regulations and guidelines. Recognize and respond to priorities in patient care with initiative and sound judgment. Demonstrate willingness to adapt to change in workload and assist with training/orienting others. Report any alleged or suspected violations to supervisor and/or others until the issue is resolved. Be self-directed, prioritize, manage time, and meet outcomes and strategic goals by targeted deadlines. Qualifications LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS: Must have a valid professional license in the state of practice. Graduate of an accredited Physical Therapy program. If foreign-trained, the program must be deemed substantially equivalent to a U.S. Physical Therapist entry-level education by a credentials evaluation organization approved by the APTA. PREFERRED SKILLS AND EXPERIENCE: Prior Home Health experience is preferable.
