We support our team with a competitive compensation and benefits package, including: Annual compensation between $125,000-$140,000, PLUS an annual performance-based, discretionary incentive. What You Will Do - Essential Functions As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care. This position allows providers to practice medicine outside of the traditional clinic setting. Providers will experience a meaningful level of clinical autonomy while maintaining a strong support structure from an interdisciplinary care team, including experienced Registered Nurses (RNs) and Medical Assistants (MAs). Please note the position requires you to meet patients in their homes throughout Maricopa County, AZ and you will be expected to cover a territory radius of 50 minutes from your home address. Mileage reimbursement is provided for eligible travel mileage exceeding 30 miles. Clinical Care & Patient Management: Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews. Manage chronic conditions and develop individualized treatment plans. Serve as the primary care provider and conduct follow-ups. Maintain accurate EMR documentation and comply with healthcare regulations. Patient Engagement & Collaborative Care Educate patients on disease management and healthy living. Collaborate with interdisciplinary teams to ensure coordinated care. Who You Are Required Qualifications: Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions. Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting. Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen. MVR with reliable transportation and clean driving record. DEA license and BLS certification Arizona market specific: 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care. Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA). Ability to document assessment findings accurately and thoroughly Maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations. Demonstrated clinical skills necessary to perform in-home assessments Working conditions: High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required. Frequent use of computer and phone systems Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations Ability to lift and carry up to 20 lb Work is performed in patient homes or facilities and may involve exposure to varied conditions. Regular travel to patient homes, provider facilities, or community locations. Employees may be exposed to varying weather conditions, road hazards, and unfamiliar environments. Mileage reimbursement is available for eligible business travel exceeding 30 miles, in accordance with company policy.
