Qualifications
Current Home Health Aide Certificate in the State of New York
Responsibilities
- Assist with patients Personal Care needs: personal hygiene, bathing, dressing/undressing, and ambulation together with any household task (eg making beds, changing linen, laundry, light housekeeping, etc) essential to the patients' health in accordance with the patient's individualized Plan of Care.
- Plan/Prepare nutritious meals and assist in feeding patients as directed. Document services provided in accurate and timely manner in accordance with company policy and personal care plan.
- Maintain Confidentiality at all times
- Report any changes in the patient's condition or incidents to RN Supervisor
- Carry out other duties
- Accompany patients to their doctor visits
