Are you a compassionate Nurse Practitioner passionate about improving patient outcomes and providing quality care to older adults?
We’re looking for a dedicated Nurse Practitioner (NP) to provide primary and acute care to nursing home residents across South Texas. This is an opportunity to make a meaningful impact through proactive patient care, chronic disease management, and timely clinical interventions.
Key Responsibilities
- Provide primary and acute care to nursing home residents.
- Perform patient assessments, diagnose conditions, and develop individualized care plans.
- Manage chronic conditions and identify changes in patients’ health.
- Collaborate with nursing staff, families, and other healthcare professionals.
- Educate patients and families on disease management and advance care planning.
- Help reduce avoidable ER visits and hospitalizations through proactive care.
Qualifications
- Master of Science in Nursing (MSN).
- Active Texas Nurse Practitioner license.
- Current NP certification.
- Active DEA registration.
- Strong clinical assessment, diagnostic, and communication skills.
- Ability to work independently and collaboratively.
- Long-term care experience preferred.
- Bilingual skills are a plus.
- New graduates are welcome to apply!
