Lead care planning and clinical oversight, guiding caregivers and supporting families with sound judgment and clear communication. Clinical Full-time / Part-time Mercer, Middlesex, and Monmouth counties Weekday schedule with rotating on-call support Open What you'll do Conduct nurse assessments and build individualized care plans Provide oversight, coaching, and support to caregiving staff Coordinate with families and medical providers Adjust care plans as client needs change and follow through Active New Jersey RN license Excellent clinical judgment and communication Commitment to responsible, person-centered care Able to perform the essential functions of the role with or without reasonable accommodation Licensing and certification Active New Jersey Registered Nurse (RN) license Preferred qualifications Home care or community-based nursing experience Experience mentoring or supervising care staff Primarily weekday hours with a rotating on-call schedule shared across the clinical team.
