Patients may be seen in: Private homes Skilled Nursing Facilities (SNFs) Assisted Living Facilities (ALFs) Board & Care settings Average caseload is approximately 4-6 patients per day. Requirements Active California Physical Therapist License Minimum 1 year Home Health Physical Therapy experience Strong independent caseload management skills Reliable transportation Why This Assignment?
